Initial Appointment

  • Anyone 5 years & above
  • Patients with Medical Assistance
  • Coordinators meet with clients weekly to work on goals
  • Children - at least 4 times a month
  • Adults - at least 6 times a month
  • Visits occurs at Home, in the Community, at the Office or at school
  • Doctor
  • Social Worker
  • School
  • Department of Social Services
  • The Court System
  • Juvenile Services
  • Health Facility
  • Church
  • Teacher
  • Counselor
  • Nurse
  • Psychiatrist/ Therapist
  • Parent
  • Legal Guardian
